Centennial Hospice, LLC

Centennial Hospice incorporates a care plan focused on both the patient and the family. We provide medical care as well as spiritual and emotional support as we focus on enhancing the quality of life of our patients and their families, and regard the passing of life as a natural process that can be greeted with dignity and respect. Centennial Hospice was formed in 2007 and serves patients and their families of central Oklahoma. Centennial Hospice exceeds both the state and national averages in Patient Care Satisfaction results, and survey results showed that 90% of all families said they would recommend Centennial Hospice to another family member or friend. Centennial Hospice has a 4:1 patient to nurse ratio, and our team includes a medical director, nurses, home health aides, social workers, chaplains, and volunteers. We work together to provide a care plan that addresses the patients'​ medical, emotional and spiritual, and social needs. - Centennial is one of five Licensed Hospices in central Oklahoma that are Joint Commission Accredited as Deemed Hospice Agency Programs. This accreditation is recognized nationwide as a symbol of quality that reflects an organization's commitment to meeting performance standards relating to quality and safety of care. - Centennial Hospice is also a 2018 recipient of the Hospice Honors recognition as a result of our high scores on quality outcome measures with the Medicare's Hospice Quality Reporting Program! We are one of only three hospice programs in the OKC-metro selected to receive this distinction, and one of only 17 selected across the state of Oklahoma.
