The Federally Qualified Health Center Urban Health Network (FUHN) is an organization of ten Minneapolis-St. Paul, Minnesota Federally Qualified Health Centers (FQHCs) who have worked together to form a HCCN (HealthCare Controlled Network) working in partnership with Minnesota’s Department of Human Services on Medicaid health care reform efforts. These efforts include FUHN’s participation in an Integrated Health Partnership Accountable Care Organization demonstration project with the State of Minnesota. Through this demonstration project FUHN participant FQHCs hope to further enhance the health care provided to their Medical Assistance patients by coupling our primary care efforts with expanded use of technology. Our demonstration project has improved service access, clinical quality and patient satisfaction/engagement, while reducing overall costs to Minnesota's Medicaid program.
