Providing excellence in hospice and palliative care as the area’s only not-for-profit community-supported hospice since 1982. More and more people are choosing to stay in the comfort of their home, whether it be their own home, a nursing facility, or a Veterans’ Home; and to live among family and friends during their final months of life. Hospice of Kankakee Valley offers the support to help make this possible by providing the services listed below and additional services if needed. These are paid for by Medicare, Medicaid, and most private insurance plans, and HKV services are provided to all who are eligible, regardless of ability to pay. Anyone can make a referral call to Hospice of Kankakee Valley with the patient’s permission. The patient or a family member can call for information, and Hospice of Kankakee Valley staff can contact the patient’s doctor to obtain records and make an assessment of the person’s appropriateness for hospice care. Hospice of Kankakee Valley patients remain under the care of their own physician, and the Hospice of Kankakee Valley team works under the direction of the patient’s physician to deliver the highest quality care to patients and families.
