Linkcare is an Integrated Care open platform allowing Health Care Professionals (Specialists, General Practitioners, Case Managers, Nurses,) to share clinical knowledge around a patient centric health care model. Linkcare has a SaaS (Software as a Service) architecture and can also be deployed "on site" and connected to a shared knowledge network. The Linkcare mobility module allows to post activities to be performed by the patients using their mobile terminal, tablet or web portal. Such activities include follow up questionnaires (PROs) and medical devices such as pulsioximeters, glucometers, scales, blood pressure and spirometers. Using Linkcare, Health Care professionals may exchange care protocols and clinical data around Integrated Practice Units, Clinical Research teams and Collaborative Community Care services. Linkcare connects with Health Informations systems such as Hospital information Systems and Electronic Health Records to update clinical data. Linkcare is an spinoff of the Barcelona's Clinic Hospital.
