Family Health Care was one of the first community health centers in the nation to develop a "one-stop shopping" concept for health care services. We've been in the business of taking care of families for almost 50 years. We offer family practice, dental, laboratory, x-ray, full service retail pharmacies, medical social work, health education, perinatal, and school-based teen health centers in Baldwin, Grant and White Cloud. Our community health outreach programs include In-Home Respite, Alzheimers Education and Referral, Teen Parent Services, the Grandparents Program, Parents as Teachers, Immunization and Well-Child Care, Behavioral Health, and Med-Read. We offer our services to patients at health centers and outreach centers in Lake, Newaygo, Wexford and Missaukee counties. OUR MISSION: Family Health Care provides quality, integrated and comprehensive health care services that are accessible to all. OUR VISION: Family Health Care (FHC) is a private, non-profit provider of choice in the communities we serve, providing 100% access to care and achieving 0% health disparities.
