CTA's Community ConNext program is a not-for-profit, 501(c)3 organization (Tax ID: 82-1489526) dedicated to providing services that instill community-wide inclusion of individuals of all abilities. Our Community ConNext program is a community-based, Applied Behavior Analysis (ABA) program that provides naturalistic opportunities to participants ages 12-22, to acquire the skills needed to connect with their communities. Our ultimate goal is to utilize evidence-based practices to offer our participants the support necessary to transition into the next stage of their lives while opening up more opportunities of choice and maximizing their independence.

Community Corrections Association Incorporated is a private nonprofit 501c3 agency which was founded in 1974. The agency operates both residential and non-residential programs in Mahoning County. There are four residential facilities at CCA totaling 217 beds. CCA also has several non- residential programs including an 8 week day reporting program serving both misdemeanor and felony offenders, a house arrest / electronic monitoring program which utilizes GPS monitoring equipment and a community service / litter patrol program in which over 10,000 hours of community service are provided annually.

The Community Council for Australia is an independent non-political member-based organisation dedicated to building flourishing communities by enhancing the extraordinary work undertaken by the charities and not-for-profit sector in Australia. CCA seeks to change the way governments, communities and not-for-profits relate to one another. It does so by providing a national voice and facilitation for sector leaders to act on common and shared issues affecting the contribution, performance and viability of NFPs in Australia. This includes: - promoting the values of the sector and the need for reform - influencing and shaping relevant policy agendas - improving the way people invest in the sector - measuring and reporting success in a way that clearly articulates value - building collaboration and sector efficiency - informing, educating, and assisting organisations in the sector to deal with change and build sustainable futures - providing a catalyst and mechanism for the sector to work in partnership with government, business and the broader Australian community to achieve positive change. Our success will drive a more sustainable and effective charities and not-for-profit sector in Australia making an increased contribution to the well-being and resilience of all our communities.

The Community Council of South Central Texas is a private non-profit Community Action Agency that serves 23 counties in South Central Texas. Counties include Atascosa, Bandera, Bee, Comal, Dimmit, Edwards, Frio, Gillespie, Guadalupe, Karnes, Kerr, Kendall, Kinney, LaSalle, Live Oak, Maverick, Medina, McMullen, Real, Uvalde, Val Verde, Wilson and Zavala. Programs operated by the agency include utility assistance, home and rental assistance, case management, disaster relief, assistance to persons with disabilities, weatherization assistance, and Women, Infants and Children (WIC). As the service delivery agency to the impoverished, CCSCT will continue to identify needs, prioritize them, and stimulate, promote, secure, and use all available resources to alleviate poverty.

Community Counseling Center of Southern Nevada promotes the healthy functioning of individuals, families, and communities through affordable, sensitive, and professional behavioral health services in a safe environment. CCC serves the majority of central Las Vegas, Henderson, and Southern Nevada rural towns.

Community Crisis Care offers commissioners, stakeholders, service users and their carers an opportunity to keep specialist in-patient stays within the community or as close to home as possible. Community Crisis Care is underpinned by the following key aims; • Managing the crisis at the point of crisis • Promoting increased connectivity with home area treatment teams, and service user’s carers by having good transport links and being centrally located • Promoting reduced lengths of stay and offering independence at the earliest opportunity by existing in the heart of communities and enabling on-going pathways • Providing specialist services that meet the needs of the community and the individual • Supporting individuals who need our care to transition into and out of our service by providing dedicated support via our transitional worker Community Crisis Care acknowledges; • Inpatient stays should be for the shortest amount of time possible • Inpatient stays form part of the overall pathway for an individual in crisis • Inpatient stays should be integrated into the community ensuring high levels of local support and maintaining as much independence as possible

The Gretchen S. Vapnar Community Crisis Center, located in Elgin, Illinois, was founded in 1975 by a group of dedicated volunteers to meet the needs of individuals and families in crisis. Employees provide comprehensive services to individuals and families in crisis due to domestic violence, sexual assault, or economic/ financial difficulties. The Crisis Center is one of the oldest domestic violence shelters in the state and was instrumental in organizing both the Illinois and National Coalition Against Domestic Violence. The mission of the Community Crisis Center is to provide Safety, Hope, Advocacy, Respite and Education for those impacted by crisis and violence in the Fox Valley Region and Illinois. Services include a 24-hour crisis hotline and information and referral; emergency temporary shelter for 40 women and children who are homeless due to domestic violence or financial crisis; individual and group counseling; medical, legal and welfare advocacy; financial assistance to abate homelessness; emergency food pantry; batterers’ intervention program; transitional living program; a distinct children’s program; and community education. The Community Crisis Center service area includes northern Kane County from St. Charles to the northern edge of the county and far northwest suburban Cook County; there are no service area limits for domestic violence victims.
