The Community Health Center exists to share the love of God by providing non-emergency medical, and eye care to those who do not have access to health care services, and assist them in establishing a continuum of care through conventional health care systems to promote a healthy body, mind and spirit. The Community Health Center mission is to ensure access to a comprehensive health care system and the delivery of quality services for the residents of Palm Beach County. The clinic is located at 2100 W. 45th Street, in West Palm Beach, Florida. That is the southwest corner at the intersection of 45th Street and North Congress Avenue. Medical Clinic Medical care for all ages is provided free of cost to those who qualify. No appointment is necessary to see a physician. Just Walk-In to our clinic during our regular hours of operation. Vision Clinic Our optometrists provide comprehensive eye exams and prescriptions for eyeglasses. They evaluate and treat many eye problems including infections, minor trauma, inflammations, glaucoma, and diabetic eye disease, using state of the art instruments and techniques. Chronic Disease Management Clinic Treating high blood pressure can take a multi-pronged approach including diet changes, medication, and exercise. Diabetes management requires awareness. Know what makes your blood sugar level rise and fall — and how to control these day-to-day factors. Diabetes is a lifelong, total-body health condition. We can help you manage your disorder, and your overall health, effectively. The Community Health Center seeks to not only meet your physical needs but also your spiritual needs. We have a chaplain on staff that prays and ministers to every patient that wishes to receive the word of the Lord. All prayers and counseling is done on a voluntary basis.

Community Health Centers, Inc. Community Health Charities is a nonprofit that raises awareness and resources for health and wellness by connecting more than 2,000 of the most trusted health charities across the United States with more than 17 million caring employees through workplace giving campaigns, causes, wellness programs, employee engagement, and strategic partnerships. Health has never been such an urgent priority: 77% of U.S. workers suffer from at least one long-term health condition, ranging from cancer to asthmachances are someone you know is affected. In the early 1940s, the federal government began to set up tables where federal employees received their paychecks. The government did this to make it simple for federal employees to make a donation to local charities that were important to them. In 1957, twelve national health charities banded together to create the National Voluntary Health Agencies (NVHA), creating what is known as a federation, a nonprofit organization that represents a category of charities through membership. Today, more than 34 federations work with the federal governments Combined Federal Campaign (CFC), which is recognized as the largest workplace giving campaign in the world. It raises more than $265 million each year, and Community Health Charities annually distributes millions of public sector contributions, making it one of the campaigns largest partners. In 1983, the idea of a federation also became appealing to corporations in the private sector who wanted to make it easy for their employees to give to local charities. Yet the administrative and clerical costs associated with setting up, managing, and maintaining relationships with dozens of charities to implement such a campaign created a significant burden. Similar to the public sector, 13 national health charities banded together to form the Combined Health Appeal® (CHA) to provide support and services to private sector corporations and conduct workplace giving. In 1998, NVHA and CHA merged, making Community Health Charities the largest workplace giving collaborative with a focus on health issues. Today, Community Health Charities is a dynamic nonprofit that raises awareness and financial resources for health and wellbeing. By focusing on the most important health issues, Community Health Charities improves liveshelping individuals, children, and familiesprovides hope and healing, and builds stronger, healthier communities.

Community Health Plan is a not-for-profit organization offering partially self-funded and fully-insured health insurance plans for employers in the Monterey Bay area. We offer the largest network of physicians and other providers on the Central Coast. We also provide Care Management services including utilization management, case management, disease management, predictive analytics, etc.
