Community Health Service Inc is a private, non-profit network of clinics providing medical, dental, behavioral health, and victim advocacy services throughout Minnesota and North Dakota. CHSI was established in 1973 to serve the health needs of migrant agricultural workers in the region, and we have expanded our scope of service in the 50+ years since. We're proud to serve the health needs of our community.

CHAD is a Primary Care Association (PCA) that takes pride in working with Community Health Centers (CHCs) in the Dakotas. CHCs are non-profit, community-driven and serve as economic engines for local economies. CHCs offer a unique model with proven results for high quality, cost-effective care customized to benefit the patient and communities being served. CHAD works with CHC members and other community leaders to find solutions for improving healthcare options in areas of the Dakotas that need it most.

Community Healthcare Center is a primary care medical, dental and counseling clinic that is focused on the patient. Our medical, dental and counseling providers are committed to providing superior primary and preventive care. OUR STORY The mission of Community Home Trust is to strengthen our community with permanently affordable homeownership opportunities for low and moderate income households. Community Home Trust sold its first home in June 2000. Today, there are more than 240 permanently affordable homes in Chapel Hill and Carrboro. Our homeowners include nurses, university employees, teachers and others, who serve our community but otherwise could not afford to own a home here. OUR SERVICES Sales: As a nonprofit, licensed real estate firm, we have three brokers on staff that provide expertise in buying and selling the affordable homes in our inventory. We provide marketing services as well as step-by-step support to get you from the contract signing to the closing table. We do not charge commission for this service. Property Management: We have staff dedicated to helping our homeowners choose a vendor to deal with repairs needed on their homes or answer questions they may have. The Home Trust manages three HOAs, for a total of 62 properties. We work closely with the HOA boards to deal with homeowner issues, governance issues, and routine and capital maintenance repairs. Education: The Home Trust provides on-going education related to our program requirements, financial counseling, homebuyer education, and home maintenance. All classes are taught by certified and trained staff and are offered at no cost to the community. Advocacy: The Home Trust uses our affordable housing experience and expertise to address affordable housing issues on the local, state, and national level. As a leader in the industry, we often provide consulting services to other non-profits and municipalities looking to expand affordable housing in their community.
