Holistic Care Hospice and Palliative Care has been serving communities throughout Southern California since 2014. We provide three types of care; Home Health Care, Palliative Care, and Hospice Care so that we may meet each person anywhere they are at with their unique healthcare needs. In everything we do, we strive to provide expert symptom management, patient-family communication, medical crisis prevention, urgent response and genuine coordination of care across medical settings to ensure continuous communication between providers during times of transition or changing needs.
