Health Promotion Council of Southeastern Pennsylvania, Inc., a nonprofit corporation, was founded in 1981 to implement community-based hypertension education and control programs. Known then as Southeastern Pennsylvania High Blood Pressure Control Program and funded by the Pennsylvania Department of Health, it was as part of a national hypertension control effort organized by the National Heart Blood and Lung Institute (NHLBI). When NHLBI disease control efforts broadened to include cholesterol and tobacco control, HPC took on these additional disease prevention concerns and changed its name accordingly. Health Quality Partners (HQP) is a not-for-profit organization that does R&D on new models of health care to enhance the effectiveness of preventive interventions and improve population health. This approach can lower the overall cost of health care for higher risk populations. This has already been convincingly demonstrated among higher risk chronically ill older adults. As tested within the Medicare Coordinated Care Demonstration and with Aetna, HQP’s model of Community-based Advanced Preventive Care, significantly reduced hospitalizations, deaths, and total Part A & B Medicare expenditures. HQP is now inviting organizations to consider becoming strategic partners and early adopters of our model. The general concept of preventive care services is not new, but a systematic program that reliably delivers a robust set of such services to higher-risk subgroups resulting in better population health and lower health care costs is a breakthrough innovation.
