Hospice of Lenawee provides compassionate, patient and family-centered care to the people of our community during and after the last season of life.

A not-for-profit locally operated hospice, HMC has provided compassionate end-of-life care for patients and their families since 1983. Patients may receive services in their own home, assisted living facility, nursing home or in one of our three hospice houses. A comprehensive hospice model, HMC is the county’s third largest healthcare company and earned Deemed Status designation from the Joint Commission, the national gold standard for quality healthcare.

Hospice of Michigan provides specialized health care services for people who are living with a terminal illness as well as services and support for their loved ones. These services, commonly referred to as palliative care, focus on comfort measures and symptom management. Palliative care is directed by the patient and family, in concert with the patient’s physician or hospice medical director. Nurses, social workers, hospice aides, spiritual care advisors and trained volunteers work as an expert team to provide care. The team focuses on providing comfort and pain relief when a cure is not possible. The goal is to help each person live as fully as possible and to help the family and loved ones participate in the person’s care.

Hospice of North Idaho has been the community's only local non-profit hospice since 1981, serving everyone regardless of their ability to pay for services. Hospice of Northwest Ohio served its first patients in 1981 and is the Toledo area's largest provider of hospice care. A non-profit, community-based organization, we are the only local agency solely dedicated to caring for the terminally ill. In 2015, we served more than 2,500 patients and their families in their own homes, in nursing homes and at our freestanding Hospice Centers. We are Joint Commission accredited and Medicare/Medicaid certified. No one is ever turned away due to inability to pay. Our mission: Hospice of Northwest Ohio provides specialized medical, emotional and spiritual care to people of all ages--and their families--living with any end-stage illness in Northwest Ohio and Southeast Michigan. Equally important, we also provide support and education to their families and caregivers.

Hospice of Santa Barbara provides care to anyone impacted by life-threatening illness or grieving the death of a loved one. Hospice of Santa Barbara is a “volunteer” hospice, which means we volunteer our professional services free of charge to meet the emotional, social and spiritual needs of people facing the end of life or grieving the death of a loved one. Each one of these people has come to Hospice of Santa Barbara with a mix of sadness and uncertainty. Hospice of Santa Barbara serves more than 700 children and adults every month. Hospice of Santa Barbara is also present on 11 local middle and high school campuses to work with children and teens who are grieving the loss of a loved one. Each one of these people has received personal, professional, wise and compassionate care. And Hospice of Santa Barbara never charges anyone for the services it offers. It does not receive insurance reimbursement or government support. All the work Hospice of Santa Barbara does is made possible by generous supporters.

Hospice of Southern Illinois is a free-standing, community-based hospice program serving 27 counties in southern Illinois, including all of the St. Louis Metro-East. With over 30 years of experience providing hospice care exclusively, Hospice of Southern Illinois is a not-for-profit corporation that provides care regardless of the patient's insurance coverage or ability to pay. Hospice is a special healthcare option for patients and families who are faced with a terminal illness. Care focuses on pain and symptom management with the goal of enhancing the quality of life. At Hospice of Southern Illinois, meeting the needs of patients and families is a priority. When is it time for hospice? 1) The patient has a terminal illness with a prognosis of 6 months or less, 2) Comfort care and symptom management become the primary focus, 3) Curative treatment is no longer the patient's choice or option.
